Ministers who have not renewed by mail postmarked January 15th, will be recorded as lapsed and in order to be reinstated must make application and pay reinstatement fees.

Download the forms below to reinstate your credentials and return the application to the district office with both release forms (if applicable), your background check disclosure form and the following checks:

    one check payable to NTD for $95.00
    one check payable to the General Council for $25.00
    one check payable to your section for $10.00
